进患者的恢复。目前的数据丧明维持治疗阶段大部分患者能很好地耐受低剂量的波尼松  龙单独使用、或泼尼松龙联合硫唑噪岭.或漫已松龙联合所畅酸酶...我们发现  仪4.5%的患者在维持治疗期间需要提前停药。将脏长期顶后不良的因素包括治疗前血机  所水平开高，病理发现新月体，治疗后血肌所升高，大量蛋白尿，高血压、慢性化评分高,男  性，非洲育和社会经济地位低下老1在追踪的过程中要监测复发的指标，如果患者的血清学指标活动度升高需要更密切的监测。目前，对 F血青学异常，但未出现临床症状时是否需要增加免疫抑制剂的剂量仍存在争议。对不同患者应根据其自身情况考虑，特别应考虑既往复发史和疾病的发展过程。我们对一组经过序贸免疫抑制治疗的中国人追踪观察了7年，复发率39.1%四，与另一组接受静味环磷酰胶诱导治疗患者的复发率相近，但是后组患者复发的时间相对较迟，缓解期的中位数79个月，这与持续3年的环磷航胺维持治疗有关，另有项研究报道，一组忠者经静脉环磷酰胺4击治疗.或甲发尼龙冲击治疗或两者联用的患者，总复发率45%.这两项研究均发现经过治疗不能完全缓解的患者浪疮的复发率高，虽然狠疮肾炎的多次复发，易引起肾衰竭,但是在我们的系列研究中无论是易复发者或不易复发者。在随诊现察中,肾功能均能维持稳定，这种表面的矛盾现象可能与发的严重程度和复发后再次缓解的速度有关。

     另一项近期的研究对比两组患者。先用拨尼松联合环磷陆胺诱导治疗。级解后一组泼尼松联合霉助酸酯维持治疗，乃组用波尼松联合每3个月静脉环磷酰肢或遗尼松联合硫唑原呤治疗.两组相比霉酚胶酷组复发半低，提示称的酸脂联合低剂量的泼尼松可用于早期维持治疗阶段。随后的霹酚酸酷的减量方法和最佳使用时间仍需研究。高血压和高血脂是很范肾炎患者常见的开发症，分别达1/3和1/5.高胆困醉血症常见于持续性蛋白尿的患者。因为这些并发症可加速浪疮患者动脉硬化.增加冠状动脉疾病的发生率，从而增加病死率和死亡率，因此需要严格的懒防和治疗1。根据报道，狼疮骨炎48%的死亡由心血管事件引发。对狼疮肾炎.ACET和ARB除降血压外，是否可保护肾力能或减少深生白尚不清楚。长期随访的狼疮肾失患者.还需监测骨贡疏松情况，注直结核等条件致向苗感染监测发生 恶性肿富的情况(特别是环磷院胺用大的患者)。另外，缓解期的患者是否可以好标及任娠的最伟时间需要仔细考息。八、小  结过去几十年来平型貌疮肾炎患者的预后已显善改善。20世纪60年代报告的5年生存率只有70%u而近报告的10 年生存幸超过90%的随着生存率的提高，长期并发症，生活质最及康复变得十分重要，虽然狼疮脑或狼疮肺死亡率仍然很高，但是很少有患者  死干象疮活动。20世纪80年代以来，免疫抑制剂有了长足的发展，而且进 步使得以往用发尼桥联合环磷胜肢获得的治疗效果现在可以用副作用更小的药物例如带的被脂代替。早期诊断和适宜的治疗对获得良好的长期预后十分重要，对于尚末解次的问题，如诱导缓解治疗和维持治疗中免疫抑制剂使用的剂量和时间，种族的差异，需要多中心的国际合作研究才能回答。
